Instructor Packets


Each instructor will have a class packet in their classroom at the start of the class. Your program manager will work with you to determine the best way to receive your packet. Generally, packets will contain the following:  

  • Class roster or registration cards used for verifying attendance 

  • Class evaluations (either a QR code for digital/electronic submission or paper copies)  

  • Certificates of completion 

  • Name tents, available upon request  

Class Rosters 

Attendance for all students must be documented on the roster. The roster may contain personal information and should not be passed around the room. Use CE’s class sign-in sheet and record attendance using the following: 

  • P= Pass  

  • I= Incomplete (student left the class early)  

  • NS= No Show 

Registration Cards  

EICC is required to keep certain student information as part of our state and federal reporting requirements. If you are not provided a roster in advance, or an attendee’s information is not included, have the student(s) complete a registration card prior to the last day of the class.  

Use the card to collect the following information:  

  • Name 

  • Date of birth  

  • Home address 

  • Daytime phone number 

  • Email (if the student doesn’t have an email, please note that on the card)  

A note about attendance 

All instructors are expected to accurately record attendance by verifying rosters or returning registration cards. Place rosters or registration cards in the packet and return it to the appropriate program manager within three business days following the last day of the class. 

Certificates  

Certificates of completion may be included in your packet or available for pick-up before the final class. If a student does not pass the class, please return the certificate to the CE office. If a student’s name is misspelled on the certificate, contact your program manager, who will mail a new one.  

Evaluations  

Class evaluations are a tool to improve the quality of our department and the class.  

Paper version: Encourage students to complete the Class evaluation. Doing so is not required.  

  1. Hand out the evaluation at the end of the last class.  

  1. Write the instructor’s name, class title and number on the board. 

  1. To ensure anonymity, ask students to stack completed evaluations in the back of the classroom. Return evaluations in your class packet. 

Electronic version: The class evaluation forms must be completed by the students at the end of your final class meeting. The QR code for the evaluation should be provided in your packet. Review the electronic evaluation form at eicc.edu/classEval or use the QR code:  

Returning the Packet 

After a completed class or training, the instructor or trainer should return or leave the packet in the designated location as arranged with the program manager. If the packet is to be left in the classroom, place the packet containing the completed roster or registration cards, and evaluations at the instructor’s station or desk. If you receive the packet electronically, please email the completed documents to your program manager.  

Court-Mandated Request for Information 

Court-mandated classes include a Request for Information form each student must complete and sign. This gives EICC authorization to report their completion to the appropriate county or state agency. Online students are emailed a secure link to the form.  

A note about court-mandated registration 

Pre-registration and pre-payment is required for all court-mandated classes. Only students who are listed on the roster can attend the course. While we understand it can be uncomfortable, please encourage any student not on the roster to register for a future class. Do not accept payment.
