A student who finds it necessary to withdrawal from the program should first confer with the program director. The program reserves the right to request withdrawal of any students whose health, work or conduct is considered detrimental to the health and safety of themselves, other students, faculty or patients. The EICC/SCC Health Information Management program reserves the right to recommend to the Dean of Students or the Dean of Instruction, the dismissal of a student for any reason, such as unsafe PPE practices, health problems that interfere with progression of the program, violation of policies or procedures, and not meeting technology requirements. Program core courses should be completed in a three year time span. Students who fail to have all required paperwork submitted to the program in order to complete their Professional Practice Experience will also be subject to dismissal from the program. The student has the right to appeal the dismissal recommendation.
If a student takes a leave of absence from the program for more than three consecutive semesters, and wishes to return to the program, there previous course progress will be re-evaluated and a new academic plan may be applied. Due to the every changing policies and procedures in healthcare there is no exception to this stipulation. Healthcare is constantly changing and we want the student to be successful, what they learned previously, may be obsolete on the National Exam. Exceptions will be dealt with on a case by case basis.

Grievance & Academic Appeal Policy
For concerns or appeals, students should first meet with the course instructor and then, if necessary, the program director, then the Division Dean. These concerns should be addressed as soon as possible. Grade appeals must be presented within 60 days from the date of which the grade was assigned. Refer to the SCC Student Handbook for additional information.
EICC Complaint Process
Located in the EICC student handbook, students are asked to follow the procedure set forth by the college for any formal complaints.

Academic and Graduation Requirements
All students’ graduation from this program must successfully complete all required program courses and achieve a 2.0 cumulative grade point average.
Students who retake a course more than two times in this program for failure to meet the C average for a course may result in a student being removed from the program.
All students must complete an “Application to Graduate” in the semester before their expected graduation.
Courses in the curriculum are sequential, for this reason students who not pass a pre-requisite course must successfully complete that course before continuing in the curriculum. Refer to the course descriptions in the college catalog for pre-requisites.
Access
Eastern Iowa Community College uses the Learning Management System Canvas, it is part of the ICCOC, which is a college consortium. You are required to take an online learning course to understand navigation through the system.
You will be issued a school email when you complete your orientation, this school email will be the only acceptable means of communication with your instructors and advisors.
